10/20/2023 EnchantaVaughn Approved (AM) Client was observed arriving to school in a jovial mood, as evidenced by his cheerful spirit as he engaged into his morning assignment. Client presented himself with a neat and clean outfit and a fresh haircut. Client was seen following directions and paying attention in class. Client was able to work well with others and showed excellent participation and communication skills during group activity. (PM) Client transitioned from classroom setting without incident. Client expressed a positive attitude throughout his entire day and showed he can handle his emotions effectively. Client communicated that he was having a good day and was awarded for his good behaviors. (EOD) Client was very well mannered toward peers, complying to rules, and attending to his assigned work. Client conducted himself accordingly in practice fire drill, standing in line and remaining quiet utilizing active listening skills when given simple instructions on proper way to exit school building safely, meeting all objectives. (AM) QMHP welcomed client to school and provided support and encouragement to ensure a good start to his day and to be on track with achieving his goals and objectives. QMHP complimented client's creative outfit. QMHP monitored client individualized behaviors in collaboration with his teacher and other mental health professional. (PM) QMHP provided praise when the client continued to display positive behaviors throughout the day, remaining on task and was very cooperative. QMHP explored client feelings and assisted client with practicing self control. QMHP challenged client to continue displaying a positive attitude while employing positive reinforcement when client is able to achieve his goals and objectives. (EOD) QMHP provided feedback to expand client's social skills while engaging him in team building exercises. QMHP assigned client to practice deep breathing and muscle relaxation during daily times of physical overactivity. QMHP encouraged client to practice aerobic exercises at home for 15-30 seconds everyday. (AM) Client greeted QMHP with a hug and said hello to everyone. Client verbalized positive feelings about goals and objectives. Client demonstrated understanding of class activities, as evidenced by his ability to sustain attention for more than 60 minutes. Client displayed good involvement with peers in class, lunch, and gym settings. (PM) Client responded in a positive manner acknowledging the positive reinforcement that the QMHP provided by saying "thank you". Client is displaying an effort to improve his ability to follow directions and work independently. (EOD) As the client has made more of his own decisions, he reported increased satisfaction and function. Client has increased his experience in making decisions, utilizing self-determination techniques. Client responded well to feedback from group members. Client agreed to practice muscle relaxation exercises at home over the weekend.
